The office strongly advises you to CALL your insurer at their customer service telephone number to verify that you will have coverage at this office. Provide them with our name and address for verification. You should also confirm that you have active coverage before coming to our office. If your plan is inactive or you are uncertain, please do not schedule an appointment unless you are prepared to pay the cost of treatment yourself. You may call us for more information on fees.
We aim to assist and provide information but ultimately we are unable to know everything about your insurance plan since there is so much variability between plans. Any treatment or portion that is not covered by insurance is the responsibility of the patient to pay. Please take an active role in understanding your insurance to avoid any misunderstandings!
